This is probably the OSD of Ffdshow. Under some circumstances ffdshow's settings get corrupted and the OSD might get turned on. To configure ffdshow, go to

Options -> Filters -> Ffdshow video decoder

and unselect OSD in the left column. You may also want to check other settings there.
